Changes to Cardholder Information
- Cardholder Name, Address Changes (Use Information/Maintenance Form)
- Credit or Transaction Limit Changes
- Leave Department Employment
- Surrender Card to Associate Vice President, Administration and Finance
Safe Guards
- Purchases are Tracked to Cardholder
- Register on USBank website using MD as the Organizations short name
- Transaction log/visa statement reviewed for appropriateness.
- Purchasing and Comptroller’s Office get automated transaction detail.
Card Security
- Use of Your Card or Your Account Number by Anyone Other Than You is Prohibited
- Secure Card and Account Number at All Times
- REMEMBER YOU ARE RESPONSIBLE FOR ALL CHARGES MADE TO YOUR CARD!!
What If You Lose Your Card, or It Is Stolen?
- Immediately Contact Jeff Canada or Vicki McGinnis
- If After Hours Contact U.S. Bank at: 1-800-344-5696 (This number is available 24 hours a day, 7 days a week) or visit US Bank Access Online website
- Complete Disputed Item Form, if requested by US Bank
- No Personal Liability if Card is Lost or Stolen
