Some of the forms below are editable "form-fill" Word documents that may be completed electronically and printed or completed by hand. The final copy must be signed prior to delivery to Environmental Safety.
Employee Incidents
All work-related incidents that result in an injury or illness must be reported to the Human Resources Office, as soon as possible. After properly attending to your injury first, please then complete the forms below and send via campus mail to the Human Resources Office, Holloway Hall, Room 153. In most cases, reports should be forwarded within 24 hours of the occurrence of the incident. If you have questions, please call (410) 543-6035.
Non-Employee Incidents
All non-employee incidents that result in an injury or illness are tracked by Human Resources. After properly attending to your injury first, please then use the form below and mail or deliver it to the Human Resources Office, Holloway Hall, Room 153, as soon as practical.
