New Student Requirements

All new students are required to submit immunization information. Each item is listed below. Please follow the link to the secure Student Health web portal to record your immunizations, and to upload your form directly into your electronic medical chart.

Failure to Complete

After completing the online requirements, you MUST submit a hard copy of EITHER a signed copy of your immunization form OR an official copy of your immunization records from your health care provider or school. If you received your immunizations in Maryland you can also access your records at the Mayland ImmuNet Website. The Tuberculosis Screening Questionnaire must still be completed online in the secure portal.

You should upload an image of your immunization form directly into the Student Health Web Portal on the immunization page.

All health forms must be completed before July 31, for the fall semester, and January 20, for the spring semester.

Immunizations and Health forms are DUE:

Fall Semester - July 31
Spring Semester - January 20th

$100 Late Fee Assessed

Fall Semester - August 31
Spring Semester - January 31

Failure to complete the requirements by August 31, for the fall semester, and January 31, for the spring semester will result in a $100.00 immunization late fee, a delay in housing check in and possible delay in course registration.

Health History

Please complete the health history form located on the secure Student Health web portal.

Please note: newly admitted students will have access to the portal once they receive information that their SU GullNet account has been converted to an SU student account.

A physical exam is not required unless you are an athlete. (Please contact Athletics for specific requirements at 410-543-6355)

Immunizations
All students are required to submit immunization data. Please download the immunization form and take it to your health care provider for completion and signature. You do not have to use our form. Any official immunization record may be submitted.

You are also required to enter immunization information and dates on the Student Health web portal.

After submitting all your information, in 2 to 3 days, you can go online to the Secure Student Health Web Portal to make sure you are compliant with all our requirements. Go to the secure portal and select “Immunizations” off the home page.

TB Screening

All students are required to complete the TB screening form located on the secure Student Health web portal. If TB risk factors are present, you must obtain a TB test within 6 months of your academic term start.

International Students

All international students on visas must have a TB test regardless of BCG vaccine. If you have had a TB skin test, blood test or chest x-ray within 6 months of arrival to Salisbury University, please provide the documentation, including the x-ray results (in English). Please do not bring the X-ray film. Insurance Information:

All students are required to submit documentation of their health insurance on the secure Student Health web portal.

Students who are under the age of 18:

To receive a vaccine or any treatment for routine injury or illness at Student Health Services, students under 18 years of age must have consent from a parent or legal guardian.
