New Student Requirements
As you prepare for your arrival at Salisbury University, there are some health related requirements that you must meet before you arrive on campus. Each item is listed below. Most information is submitted by students via the secure patient portal. Please do not hesitate to contact Student Health Services should you have questions at 410-543-6262.
Failure to Complete
After completing the online requirements, you MUST submit a hard copy of EITHER a signed copy of your immunization form OR an official copy of your immunization records from your health care provider or school. Students that have served in the Military, may provide their Military ID as proof of immunizations. The Tuberculosis Screening Questionnaire must still be completed online in the secure portal.

All health forms must be completed before July 31, for the fall semester, and January 20, for the spring semester.
Immunizations and Health forms are DUE:
- Fall Semester - July 31
- Spring Semester - January 20th
$100 Late Fee Assessed
- Fall Semester - August 31
- Spring Semester - January 31
Failure to complete the requirements by August 31, for the fall semester, and January 31, for the spring semester will result in a $100.00 immunization late fee, a delay in housing check in and possible delay in course registration.

Please note: newly admitted students will have access to the portal once they receive information that their SU GullNet account has been converted to an SU student account.
A physical exam is not required unless you are an athlete. (Please contact Athletics for specific requirements at 410-543-6355)
All students are required to submit documentation of their health insurance on the secure Student Health web portal.
Students who are under the age of 18:
To receive a vaccine or any treatment for routine injury or illness at Student Health Services, students under 18 years of age must have consent from a parent or legal guardian.
