Incident Reporting

All work-related incidents that result in an injury or illness must be reported to Environmental Safety / Human Resources. After properly attending to your injury, please use the forms below and fax (75026), mail or deliver them to Human Resources (HH 153) as soon as practical.  Reports should be forwarded to Human Resources within 24 hours of the occurrence of the incident.

Some of the forms below are editable "form-fill" Word documents that may be completed electronically and printed or completed by hand. The final copy must be signed prior to delivery to Human Resources.

All non-employee incidents that result in an injury or illness are tracked by Environmental Safety. After properly attending to your injury, please use the form below and fax, mail or deliver it to Environmental Safety (MB 100A) as soon as practical.
