Dual Employment Form (Word) Form required by multiple supervisors before employee can work/earn funds from two distinct funding sources. Position Description (PD) Form (Word)
Form needed to collect information about a new exempt staff position. This form is required when submitting a new employment contract.
Position Information Form (Word)
Form needed to collect information about a new non-exempt staff position. This form is required when submitting a new employment contract.
I9 Certification Form (PDF) - listed under Recruitment/New Hire Process
Required for all new employees or employees with a break in service (to be completed on first day of employment).
W-4 Form (PDF)
State of MD required tax form in order for setup on payroll systems and to update /change personal deduction amounts for federal and state withholding.
Direct Deposit Form (PDF) Form needed by State of MD to initiate direct deposit for earning directly to checking account.
SU Policy Sign-Off Sheet (Word)
Required for all new employees or employees with a break in service (acknowledges receipt of State of Maryland Substance Abuse Policy, SU Sexual & Other Discriminatory Harassment Policy and SU Procedures for PGSRuing Complaints of Sexual & Other Discriminatory Harassment).
Employee Data Sheet (Word) - listed under Employment Contracts
Required for all new employees or employees with a break in service.
[The above listed paper forms are required to be completed and processed for any grant/contract related activity employing staff. Currently, the GSR-FGMO completes the appropriate forms and meets with newly hired employees for form(s) signature and explanation of contract and all other related benefits and paperwork.]
