WELLNESS NEEDS AND INTEREST SURVEY FOR CAMPUS COMMUNITY
As you may be aware, President Janet Dudley-Eshbach has sponsored the establishment of an Employee Wellness Program managed by the Office of Human Resources. SU Wellness Coordintor Laura Hemp works closely with the Wellness Committee, made up of representatives from across the campus, to develop and implement programs and offerings to serve employees. However, we would like to hear directly from you. Please take 5-10 minutes to answer a series of questions to provide feedback and guide our campus health and wellness programs.
As the concept of Employee Wellness may mean different things to different people, the purpose of this survey is to obtain your input and/or comments that will assist the Employee Wellness Committee in determining which wellness activities are most important and are of interest to you. There are a wide variety of programs and initiatives that can be introduced, offered and/or developed through an Employee Wellness program. However, the main focus should be to encourage and motivate employees to take a proactive approach on their own health and well-being, at work and in their personal lives.
At the end of the survey you will be able to make yourself eligible to win one of the THREE drawings for prizes. Prizes include
one free month at the
University Fitness Center OR
one free Aquamassage at the Personal Wellness Center.
Thank you for your feedback and participation.
