Instructors may request library instruction for a class by one of two methods:
  1) Contact the library liaison for your department directly to make the arrangement.
  2) Submit
information in this form.  Complete the form once for each class requested. 
 
Instruction requests should be submitted at least two weeks in advance of your preferred date.  Instructors are expected to attend each instruction class.
