If you do not have a Passport, you will need to get one before you go out of the country and turn in a copy as soon as possible.

To get a passport, you will need to:
ü Provide an application form, DS-11
o Can be obtained from U.S. Department of State - Passport Application

o From any passport agency or acceptance facility

DO NOT SIGN THE DS-11 APPLICATION UNTIL THE PASSPORT ACCEPTANCE AGENT INSTRUCTS YOU TO DO SO.
ü Present proof of U.S. Citizenship, in the form of one of the following:
o Previous U.S. Passport (mutilated, altered, or damaged passports are not accepted)
o Certified birth certificate
ü Present Proof of Identity, in the form of one of the following:
o Previous U.S. Passport (mutilated, altered, or damaged passports are not accepted)
o Naturalization Certificate
o Current, valid
§ Driver’s license
§ Government ID, city, state ,or federal
§ Military ID: military and dependents
ü Have two passport photos; requirements:
o 2x2 inches in size
o Photos must be identical
o Color
o Full face, front view
These can be done at the Post Office on Rt. 50, Kinko’s, or Eckerd’s Drug
ü Pay the applicable fee
o Application fee: $67
§ Personal checks, money orders, and bank drafts at all locations
§ Exact cash at some locations
o Execution fee: $30
§ Money orders and bank drafts at all locations
§ Personal checks and exact cash at some locations
§ Credit cards at U.S. Postal Facilities and some other locations
o Expedited Service fee: $60 for each application
Please allow at least 10 weeks for delivery of your Passport. If you will need it sooner than 10 weeks, you will need to have your passport expedited.
