Survey of Faculty Extra-Instructional Activity

For the Period: June 1, 2004 - May 31, 2005 

(Including Summer 2004)

Please complete the following questionnaire and submit it electronically to the Office of Institutional Research.  

This year, the Deans are asking that you use your name when completing the form.  Your individual responses will be disclosed ONLY to your department chair and your academic dean.  All other reports, whether internal or external, will aggregate your information with that of your colleagues.  

Immediately after you "Submit" the survey, you will receive a printout of your responses. You may want to place a copy of that report and any supporting materials in your academic portfolio. 

Please remember to identify yourself on the survey.  The survey deadline is  June 15, 2005.


Please note that many of the questions ask for approximations. For those questions, an educated estimate is all that is needed--you do not need to be precise.  Also, to avoid additional work on your part, we do not ask for information (for instance, teaching and advising loads) that we are collecting through other means.
