In response to an
USM discussion to pass the cost of this annual survey back to individual
campuses, the Survey of Faculty Extra-Instructional Activity (previously
titled Non-Instructional Productivity) is being collected via the web.
Please complete the entire form and submit it electronically to the Office of
Institutional Research. Your
responses will be aggregated along
with those of your colleagues and submitted to your department chair and Dean
and, as per USM directives, to the Board of Regents. Since
the Office of Institutional Research will be processing surveys by a
"Survey ID Code" (given to you by the department secretary or
chair), the identity of respondents is confined to the departmental level.
Immediately after you "Submit" the survey you will receive a printout of your responses. You may want to place a copy of that report and any supporting materials in your academic portfolio.
We need 100% participation to accurately demonstrate the depth and breadth of SU faculty extra-instructional activity. In order for us to track participation accurately, please identify yourself on the survey with your assigned "Survey ID Code." Your code is the four-letter prefix of your department, for instance PSYC for Psychology AND the number assigned to you by your department secretary or chair. In order to track and foster participation, department chairs will receive regular updates of respondents listed by "Survey ID Code." Surveys submitted without an appropriate "Survey ID Code" cannot be processed. Please respond no later than May 31, 2004.
