Student Health Services

New Student Requirements

As you prepare for your arrival at Salisbury University, there are some health related requirements that you must meet before you arrive on campus. Each item is listed below. Most information is submitted by students via the secure patient portal. Please do not hesitate to contact Student Health Services should you have questions at 410-543-6262.

After completing the online requirements, you MUST submit a hard copy of EITHER a signed copy of your immunization form, OR, an official copy of your immunization records from your health care provider or school.

Failure to complete the requirements by August 31, for the fall semester, and January 31, for the spring semester will result in a $100.00 immunization late fee, a delay in housing check in and possible delay in course registration.

Health History:

Please complete the health history form located on the secure Student Health web portal at myhealth.salisbury.edu.

Please note newly admitted students will have access to the portal once they receive information that their SU GullNet account has been converted to an SU student account.

A physical exam is not required unless you are an athlete. (Please contact Athletics for specific requirements at 410-543-6355)

Immunizations:

All students are required to submit immunization data. Please download the immunization form and take it to your health care provider for completion and signature.

 You are also required to enter immunization information and dates on the Student Health web portal at myhealth.salisbury.edu.

After submitting all your information, in 2 to 3 days, you can go online to the Secure Student Health Web Portal to make sure you are compliant with all our requirements. Go to the secure portal and select “Immunizations” off the home page.

TB Screening:

All students are required to complete the TB screening form located on the secure Student Health web portal at myhealth.salisbury.edu. If TB risk factors are present, you must obtain a TB test within 6 months of your academic term start.

International Students:

All international students on visas must have a TB test regardless of BCG vaccine. If you have had a TB skin test, blood test or chest x-ray within 6 months of arrival to Salisbury University, please provide the documentation, including the x-ray results (in English). Please do not bring the X-ray film. If you have not had a TB test, please have a Quantiferon Gold Test or T-Spot blood test done and submit documentation via fax: 410-548-4101, email: studenthealth@salisbury.edu or mail, Salisbury University Student Health Services, 1101 Camden Ave, HH 180, Salisbury, MD 21801

Insurance Information:

All students are required to submit documentation of their health insurance on the secure Student Health web portal at myhealth.salisbury.edu.

Students who are under the age of 18:

To receive a vaccine or any treatment for routine injury or illness at Student Health Services, students under 18 years of age must have consent from a parent or legal guardian. Consent for Treatment:

Please complete the consent for treatment located on the secure Student Health web portal at myhealth.salisbury.edu.

Please complete the privacy practices located on the secure Student Health web portal at myhealth.salisbury.edu. Please protect your log in and password information.
