PART B
Final Report questionnaire: trainee self-assessment

Background Details:
In line with guidelines specified by the federal government in relation to the ACE project, your final report will consist of two parts. The guidelines for Part A have already been mailed to you. This is the final part. This was to be sent out in early January but we were waiting on an NCATE meeting whose results are incorporated into this questionnaire. Both parts of the report: Part A and B are due on or before March 31, 2004.
Instructions
Section I and II of this form involve simply circling your responses. Section III of the form requires you to fill in your choices. The entire form should not take more than 15-20 minutes of time. Please mail this completed form with your circled response with Part A of the report.
