Application for Educational Benefits

Post 9/11 (Chapter 33) and Montgomery G.I. Bill students need to start by going online to the VA website and filling out the Application for Educational Benefits (Form 22-1990).  To access the application, go to

In order to receive Post 9/11 benefits students will need to submit a Veteran Benefits Certification Request. Upon verification, you will receive a certificate of eligibility stating the percentage of benefits you qualify for, and the number of months you are eligible to collect benefits.

Survivor’s and Dependent’s Education Assistance (Chapter 35)

Students who are dependents of service members who were severely wounded or died as a result of injuries received in the line of duty will need to fill out the Survivors’ and Dependent’s Educational Assistance form (Form 22-5495), which is available online at

Military Experience

As a veteran, please submit your AARTS, SMARTS or USCGI transcripts (as applicable), and DD214 to the Admissions Office.  You can order your military transcripts at:

Transfer Students

If you have recently transferred from another Institution, or have completed your undergraduate degree and wish to pursue a graduate degree: complete the VA Form 22-1995, which is available online at

Satellite Students

All courses offered at satellite locations are coded for the main Salisbury, MD location.  Salisbury University is a single location employer.  Housing allowance payment will be based on area code 21801.  Additionally, many of the courses held at our satellite locations will be coded as distance education based on Salisbury Universities hybrid policies.
