First Time Applicants for Educational
Benefits:
- Provide a COPY of your DD-214 or
Notice of Benefit Eligibility to the
Registrar's Office.
- Provide a copy of all kickers, completed
certifications, college credits or MCI's.
This will allow us
to process your benefits faster and
more efficiently.
- Chapter 1607 must request Reservist
benefits specifically.
- Complete an application for Educational
Benefits Form 22-1990 or 22-5490
(Chapter 35 Dependents only) which we
provide in our office. This can also
be completed online at the VA website,
www.gibill.va.gov/apply-for-benefits/application/
but you must provide us with a copy.
If you have used your benefits at another
institution:
- Provide a COPY of your DD-214 or
Notice of Benefit Eligibility to the
Registrar's Office.
- Provide a copy of all kickers, completed
certifications, college credits or MCI's.
This will allow us
to process your benefits faster and

- Complete Form 22-1995 or
22-5495 (Chapter 35 Dependents only)
Change of Program or Place of Training,
available in our office. This can also be
completed online at the VA website,
www.gibill.va.gov/apply-for-benefits/application/
but you must provide a copy to us.
Once you have applied
for your benefits:
- Submit a
Request for Certification for VA Benefits
(pdf) to the Registrar's Office each
semester. Certification will not be
processed without this document.
- Contact VA (see
VA
website for instructions) to verify your
enrollment each month.
- If you change your major or program, you
must submit either a Form
22-1995 or 22-5495 (Chapter 35
Dependents only).
This form needs to be
filed online at the VA website,
www.gibill.va.gov/apply-for-benefits/.
Please provide us with a copy for our
records.
- If you increase or
decrease the number of credits you are
taking you must notify the Registrar's
Office as soon as possible. Failure
to report changes may affect your benefit
payment and future eligibility.
Special
Instructions for Chapter 33
- In addition to the
forms outlined above, please provide a copy
of your Certificate of Eligibility stating
the percentage of benefits to which you are
entitled under the Post 9/11 GI Bill.
- If switching to the
POST 9/11 GI Bill from a different chapter
of benefits, please re-submit an Application
for Benefits (Form 22-1990) to the VA,
electing to switch to Chapter 33. This is
an irrevocable decision. For more

- Chapter 33 does not
require monthly verification of attendance
with VA Once.

Readmission
Requirements for Service Members
A student who is called to
active duty in the United States Armed Services,
the National Guard or Reserves is entitled to
readmit to Salisbury University with the same
academic status held at the time of separation
if all of the following criteria are met:

The student has not attended
and been
academically dismissed from
another
institution prior to the request for
readmission to Salisbury University.

The student must complete a Readmission
Application to notify the Registrar’s
Office of their intent to readmit. If the
absence was for more than one calendar year,
a Residency
form must also be submitted. Both forms can
be accessed at
www.salsibury.edu/registrar/forms.

A Request for Certification for VA
Eduactional Benefits may be
completed if the student is eligible for
these benefits. See
